Nurse Case Manager PRN
Job Requirements
Responsible for coordinating resource utilization for patients accessing care through Upper Chesapeake Health. Performs utilization review for all inpatients. Works collaboratively with inter-disciplinary team to assess plan of care, monitor progress and make recommendations the safest and most efficient hospitalization is achieved. Works with social work to assist with discharge planning. May do emergency room evaluations to cover absences of ED case manager. Night Shift Care Manager will do emergency room evaluations for psychiatric patients. Applies knowledge of diagnostic criteria to evaluate and recommend to the appropriate level of care for the behavioral health patient. Planning, organizing, and directing case management activities. May interact with patients or customers ranging in age from newborn to geriatric.
Work Experience
Current Maryland RN license required. Completion of a Bachelors of Science degree in Nursing required (or in progress). Masters of Nursing preferred. Experience in Psychiatric Case Management, Utilization Management and Critical Pathways preferred. Basic computer skills required. Demonstrate skill in a) psychiatric clinical case management and assessment of psychiatric patients who may present in the emergency department; b) performing complete biopsychosocial assessments; c) effective critical thinking; d) written and oral communications; and e) age appropriate interpersonal interactions (patients may range from newborn to geriatric adult). Demonstrate knowledge of a) Medicare and Medicaid regulations; b) acute care criteria and c) case management principles. Demonstrate ability to a) communicate and collaborate effectively with both internal and external customers (colleagues, Medical Staff, liaisons, and patient/family); b) assess, adapt, and calmly respond to changing and/or crisis environment; c) make independent decisions consistent with current policies, procedures, and ethical standards; d) prioritize work assignments and manage time effectively to complete duties; and e) be available to adapt hours to meet the needs of all customers. Requires knowledge and use of the appropriate computer systems to communicate effectively with both internal and external customers.
